Sports Medicine & Performance Rehabilitation
Led by Dr. Fabian Azofeifa PT, DPT, our sports rehabilitation program combines his expertise as a certified CrossFit Level 1 Trainer with advanced physical therapy techniques. With a strong athletic background including collegiate soccer and Olympic weightlifting, Dr. Fabian provides specialized care for athletes and CrossFit enthusiasts. We treat sports injuries, post-surgical recovery, and performance optimization using manual therapy, movement analysis, and progressive exercise protocols tailored to each individual's athletic goals and training demands.
Manual Therapy & Pain Management
Our manual therapy and pain management program combines the expertise of both Dr. Fabian and Dr. Susannah Azofeifa, offering over 10 years of combined experience in treating chronic pain conditions. We utilize advanced manual therapy techniques, dry needling, and evidence-based interventions to address the root causes of persistent pain. Horizon Physical Therapy & Wellness is led by Dr. Fabian and Dr. Susannah Azofeifa, co-founders with over 10 years of combined experience in physical therapy and rehabilitation. Our clinic specializes in orthopedic rehabilitation, women's health, vestibular rehabilitation, and manual therapy, providing personalized care for patients of all ages. Dr. Fabian brings expertise in sports medicine, performance rehabilitation, vestibular rehabilitation, TMJ rehabilitation, and manual therapy techniques, while Dr. Susannah focuses on women's health, general orthopedics, and geriatrics. Our Patient Care Coordinator, Daniela Espaillat, ensures every patient receives compassionate, comprehensive care throughout their recovery journey.
