From my soccer days and Olympic weightlifting career, I came to understand just how deeply an athlete longs to recover from an injury and return to the playing field. I also believe every patient should be treated the way I would want to be treated if I visited a medical office—with respect, in a timely manner, and without cost surprises. We strive to deliver on all those fronts.

My journey toward becoming a physical therapist began with my lifelong love of soccer and exercising. Playing sports and being active since childhood taught me the value of movement, discipline, and resilience. I saw firsthand how crucial physical health is—not just for performance, but for overall well-being. It wasn't until my grandmother suffered a stroke in 2010 that I truly understood the power of physical therapy.

Watching her go through rehabilitation gave me a deeper, more personal perspective on recovery. I saw how physical therapists didn't just help her regain strength and mobility—they helped restore her confidence and independence. Transitioning from the hospital, to a nursing home and eventually home living alone and independently. That experience left a lasting impact on me. It showed me how physical therapy can truly transform lives.

I want to be part of that process for others that are suffering with physical ailments—supporting, guiding, and empowering them through recovery and movement, just as my grandmother's PTs did for her.

Fabian's Family

Fabian's Journey Embodies the American Dream—a story of determination, resilience, and unyielding commitment to success. Born and raised in Costa Rica, Fabian arrived in the United States as a nine-year-old in 2002. Soccer, CrossFit, physical therapy school, and a strong foundation of faith and family have shaped his life. Soccer injuries changed his career path, inspiring him to help others achieve full recovery through physical therapy. He went on to earn his Physical Therapy degree from Brenau University, where he met his wife and co-owner of Horizon Physical Therapy & Wellness, Susannah Azofeifa.

Arriving in a new country, Fabian faced the challenges of adapting to a different culture, language, and way of life. Rather than being deterred by these obstacles, he fell in love with the promises of what America offers—the chance to create something from nothing, the belief that you are the main obstacle to your own success, and the conviction that with God's help, all things are possible.

Fabian's love for the United States grew over the years, and he feels ever grateful for what this country has given him. That sense of gratitude, along with his Christian beliefs, drives him to give to others—especially the less fortunate, or in this case, the injured.

After working in private practice in the CSRA for five years, Fabian sought to provide quality physical therapy that focuses on the individual and not just the masses. With little more than a vision and a relentless work ethic, Fabian and Susannah will open the doors to Horizon Physical Therapy & Wellness in the fall of 2025—a business built from the ground up.

Fabian and Susannah don't just want to be physical therapists; they want to positively impact lives—helping patients recover, regain independence, and live pain-free. From humble beginnings to becoming a small business owner, Fabian's story is a testament to what this country offers. He came to the United States with very little but a willingness to work hard, determination, and resilience—and is creating a life he once only dreamed of.
